Distal-to-proximal progression

Begin with the most distal acceptable site and move proximally if access fails or therapy needs change. This preserves upstream veins. Jumping first to an antecubital or upper-arm site for a brief peripheral therapy can “burn” a future PICC pathway or eliminate backup sites.

Avoid joint flexion areas for ongoing therapy

Sites over the wrist crease or antecubital fossa experience continuous mechanical motion. Motion increases:
