Three components, not one
IJCAHPO is explicit: COMT certification requires successful completion of the multiple-choice examination, the COT Skill Evaluation, and the COMT Performance Test. Candidates who are already COT certified and have previously completed the COT Skill Evaluation are not required to repeat it — which is why the COMT-TG3 and COMT-TG5 pathways are attractive for working technicians.
Both practical components are computer-simulated. You are not handed a phoropter and a live patient; you work through a simulated clinical task on screen. That format rewards knowing the correct sequence and the correct recording convention as much as the physical technique.

The seven COT Skill Evaluation tasks
- Lensometry — perform non-automated lensometry to determine the strength of the distance correction and the bifocal or trifocal add. The task may be performed in plus or minus cylinder.
- Visual fields — perform an automated visual field on a specified automated perimeter as determined by IJCAHPO.
- Ocular motility — detect a phoria or a tropia and identify the direction of the deviation using appropriate cover tests.
- Keratometry — perform keratometry.
- Retinoscopy — perform retinoscopy, in plus or minus cylinder.
- Refinement — perform refinement, in plus or minus cylinder.
- Tonometry — perform applanation tonometry.
Notice that four of the seven (lensometry, retinoscopy, refinement, keratometry) sit in content areas the written exam weights lightly — 4%, 6% and 2%. The practical restores their importance. A candidate who dismisses manual lensometry as "old technology" will meet it twice: once on the Skill Evaluation and again on the Performance Test.
The five COMT Performance Test tasks
Candidates completing the computer-simulated COMT Performance Test are asked to demonstrate skill in exactly five areas:
- Measure the patient's ocular motility using prism and cover tests at distance.
- Perform manual lensometry: identify and measure prism.
- Perform fundus photography and identify fluorescein angiography phases.
- Measure, compare and evaluate pupil function at distance.
- Evaluate versions and ductions, identifying any abnormalities.
Read that list against the written blueprint and the design intent is obvious. Three of the five tasks are motility or pupil tasks — the same areas the written exam weights at 12% and 3%. One is manual lensometry with prism. One is fundus photography plus fluorescein phase identification, which is where the 9% imaging and 8% photography weights converge.
Study consequence: the highest-leverage preparation for the COMT is not reading about exotic pathology. It is being able to (a) run a distance prism-and-alternate-cover measurement and record it correctly, (b) neutralise a lens with prism on a manual lensometer, (c) name the fluorescein phase from an image, and (d) compare pupils in light and dark and quantify an afferent defect.
Sequencing, timelines and attempt limits

Work the checklist literally: it is the scoring rubric in disguise. For the Skill Evaluation, the optional practice examination is the only sanctioned rehearsal of the simulation interface, and for candidates who have never used a computer-simulated practical it is usually worth the fee.
Building a rehearsal plan for the practicals
Because both practical components are computer-simulated and neither the COMT multiple-choice examination nor the Performance Test has a sanctioned practice version, rehearsal has to be built from the published task lists. Work each task as a written protocol before you work it on a machine.

A useful self-check is to write the recording line you would enter in the chart for each task. If you cannot write the line, you do not yet know the task well enough to be scored on it.
