- Arises within the first 2 to 10 weeks of life due to transplacental transfer of maternal androgens stimulating infant sebaceous glands.
- Manifests as thick, tenacious, greasy, yellowish-brown crusts and scales over the vertex of the scalp. Lesions may spread to the face, retroauricular folds, and diaper region.
- Benign & Self-Limiting: The infant is asymptomatic (does not scratch; non-pruritic). It resolves spontaneously by 6 to 12 months of age.
- Management: Reassurance; conservative measures include applying warm mineral oil, baby oil, or petrolatum to the scalp to soften crusts for 20 minutes, followed by gentle mechanical desquamation with a soft-bristled baby toothbrush and washing with a mild baby shampoo. For persistent inflammation, a low-potency topical steroid (hydrocortisone 1% cream) can be applied for several days.

- HIV Infection: While mild seborrheic dermatitis occurs in 3–5% of the general population, it occurs in up to 85% of individuals with advanced HIV/AIDS. An explosive, sudden-onset, widespread, fiery-red, treatment-refractory seborrheic dermatitis involving the entire face, neck, and trunk is a classic early clinical harbinger of acute HIV seroconversion or progressing immunosuppression (CD4 count <400/μL). Mandatory Board Rule: Any young or middle-aged adult presenting with sudden, extensive, recalcitrant seborrheic dermatitis must be screened with a fourth-generation HIV-1/2 antigen/antibody immunoassay.
- Parkinson Disease & Neurodegenerative Disorders: Seborrheic dermatitis exhibits an extraordinary prevalence in Parkinson disease. Autonomic nervous system dysfunction induces excessive sebum production ("seborrhea") and pooling, coupled with immobility of facial expression (the parkinsonian "masked facies"), creating a fertile environment for Malassezia overgrowth.

Longitudinal Management Strategy
Because seborrheic dermatitis is a chronic condition with an intrinsic genetic and microbial basis, the therapeutic goal is symptom control and maintenance, not permanent eradication.
- Scalp Disease (Adult Dandruff & Plaque Disease):
- Medicated Antifungal Shampoos: Ketoconazole 2% shampoo, Selenium sulfide 2.5%, or Zinc pyrithione 1–2%. Patients must lather the shampoo into the scalp and leave it in place for 5 to 10 minutes before rinsing to ensure adequate fungicidal tissue penetration. Use 2 to 3 times weekly.
- Topical Corticosteroid Solutions/Foams: For acute intense pruritus and erythema, mid-potency topical corticosteroids (fluocinolone acetonide 0.01% scalp oil or clobetasol 0.05% shampoo used twice weekly) provide rapid symptomatic relief during flares.
- Facial and Delicate Skin Folds:
- Topical Antifungals: Ketoconazole 2% cream or ciclopirox 0.77% cream applied once to twice daily is the first-line maintenance therapy of choice, directly suppressing Malassezia load without skin-thinning side effects.
