1A 24-year-old primigravida presents for her initial antenatal visit. She reports that her last normal menstrual period (LMP) began on May 14, 2026, and she has regular 28-day cycles. Using Naegele's rule, what is her estimated date of delivery (EDD)?
A.March 7, 2027
B.February 14, 2027
C.February 28, 2027
D.February 21, 2027
Explanation: According to Naegele's rule, the estimated date of delivery is calculated by adding 7 days to the first day of the last menstrual period, subtracting 3 months, and adding 1 year. For an LMP of May 14, 2026: May 14 + 7 days = May 21; subtracting 3 months gives February 21; adding 1 year results in February 21, 2027.
2During an abdominal examination of a woman at 36 weeks gestation, the midwife performs the first Leopold maneuver (fundal grip) and palpates a broad, soft, irregular, and non-ballotable mass in the uterine fundus. What fetal presentation does this finding indicate?
A.Breech presentation
B.Shoulder presentation (transverse lie)
C.Cephalic presentation
D.Face presentation
Explanation: In cephalic presentation, the fetal breech occupies the uterine fundus. The breech palpates as a broad, soft, irregular, and non-ballotable pole, whereas the fetal head is hard, round, smooth, and easily ballotable. Because the breech is in the fundus, the head is presenting at the pelvic inlet, confirming cephalic presentation.
3Which of the following maternal cardiovascular physiological adaptations is considered normal during an uncomplicated singleton pregnancy?
A.Decrease in resting heart rate by 10 to 15 beats per minute
B.Marked elevation in systemic vascular resistance during the second trimester
C.Increase in cardiac output by 30% to 50% above pre-pregnancy levels
D.Significant reduction in total plasma volume leading to hemoconcentration
Explanation: Normal maternal cardiovascular adaptations include an increase in cardiac output by 30% to 50%, peaking in late second and early third trimesters. This is achieved through increased stroke volume and an elevated resting maternal heart rate of 10 to 15 bpm. Systemic vascular resistance decreases due to progesterone-mediated vasodilation, and plasma volume expands by 40% to 50%.
4A 35-year-old multigravida undergoes second-trimester quadruple serum screening at 16 weeks gestation. Which maternal serum biochemical marker pattern is most suggestive of fetal Down syndrome (Trisomy 21)?
A.Elevated maternal serum alpha-fetoprotein (MSAFP) and elevated unconjugated estriol (uE3)
B.Decreased hCG, decreased inhibin A, and elevated MSAFP
C.Elevated human chorionic gonadotropin (hCG) and elevated inhibin A, with decreased MSAFP and decreased uE3
D.Normal hCG, elevated MSAFP, and normal inhibin A
Explanation: The quadruple screen for Down syndrome (Trisomy 21) characteristically demonstrates elevated levels of human chorionic gonadotropin (hCG) and dimeric inhibin A, accompanied by decreased levels of maternal serum alpha-fetoprotein (MSAFP) and unconjugated estriol (uE3). Elevated MSAFP is associated with neural tube defects or abdominal wall defects.
5An Rh-negative, unsensitized primigravida (antibody screen negative) presents at 28 weeks gestation. Which clinical intervention is standard practice to prevent Rh(D) alloimmunization?
A.Perform immediate diagnostic amniocentesis to determine fetal blood group antigens
B.Withhold anti-D immune globulin until immediately after delivery regardless of maternal antibody status
C.Administer oral dexamethasone to downregulate maternal antibody response
D.Administer 300 mcg (1,500 IU) of anti-D immune globulin intramuscularly
Explanation: Standard antenatal care requires administering 300 mcg (1,500 IU) of Rh(D) immune globulin intramuscularly to all unsensitized Rh-negative pregnant women at 28 weeks gestation. A second dose is administered within 72 hours of delivery if the infant is confirmed to be Rh-positive. This substantially decreases the risk of maternal isoimmunization caused by silent fetomaternal hemorrhage.
6A 29-year-old woman undergoes a 75 g 2-hour oral glucose tolerance test (OGTT) for gestational diabetes screening at 26 weeks gestation. According to WHO criteria, which plasma glucose result confirms the diagnosis of gestational diabetes mellitus (GDM)?
A.Fasting venous plasma glucose of 4.4 mmol/L
B.1-hour venous plasma glucose of 8.2 mmol/L
C.2-hour venous plasma glucose of 8.8 mmol/L
D.Fasting venous plasma glucose of 4.9 mmol/L
Explanation: Under WHO and IADPSG criteria, gestational diabetes is diagnosed if one or more venous plasma glucose values meet or exceed: Fasting >= 5.1 mmol/L (92 mg/dL), 1-hour >= 10.0 mmol/L (180 mg/dL), or 2-hour >= 8.5 mmol/L (153 mg/dL). A 2-hour plasma glucose of 8.8 mmol/L exceeds the 8.5 mmol/L threshold and establishes the diagnosis of GDM.
7A midwife measures the symphysis-fundal height (SFH) of a pregnant woman with an uncomplicated singleton pregnancy at 28 weeks gestation. Which measurement is within normal expected clinical limits using McDonald's rule?
A.21 cm
B.28 cm
C.34 cm
D.18 cm
Explanation: Between 20 and 36 weeks gestation, the symphysis-fundal height in centimeters approximates the gestational age in weeks within a margin of plus or minus 2 to 3 cm. At 28 weeks, an SFH of 28 cm is exactly on target (normal range 26 to 30 cm). Measurements discordant by more than 3 cm warrant ultrasound evaluation for fetal growth restriction, macrosomia, oligohydramnios, or polyhydramnios.
8A midwife teaches a nulliparous woman at 32 weeks gestation a daily 'count-to-ten' fetal movement method: resting on her side, she notes how long it takes to feel ten distinct movements. What should the midwife advise about a reassuring count?
A.Fetal movements naturally cease entirely after 32 weeks due to limited intrauterine space
B.She must feel at least 10 movements within 24 hours regardless of activity
C.Any perception of fewer than 30 movements in 1 hour warrants immediate emergency delivery
D.She should perceive at least 10 distinct movements within a maximum of 2 hours while resting quietly
Explanation: In the count-to-ten approach the woman rests on her side in a quiet setting and counts distinct movements (kicks, flutters, rolls). Feeling at least 10 movements within 2 hours is reassuring; the original Cardiff chart allowed a longer window, but many services now use the 2-hour count. Fewer than 10 movements in that time, or a clear reduction from the usual pattern, needs prompt assessment (CTG and ultrasound).
9A cardiotocograph (CTG) non-stress test (NST) is performed on a 34-week gestation fetus. Over a 20-minute tracing, the baseline heart rate is 140 bpm with moderate variability, and there are two heart rate accelerations of 16 bpm above baseline lasting 20 seconds. How should the midwife interpret this tracing?
A.Non-reactive, requiring prolonged 40-minute acoustic stimulation
B.Equivocal, indicating impending fetal acidemia and requiring immediate cesarean delivery
C.Reactive (reassuring), indicating fetal autonomic competence and adequate oxygenation
D.Pathological, demonstrating severe fetal cardiac rhythm disturbance
Explanation: A reactive non-stress test at >= 32 weeks gestation requires a normal baseline rate (110-160 bpm), moderate variability (6-25 bpm), and at least 2 accelerations that peak at >= 15 bpm above baseline and last for at least 15 seconds from onset to return within a 20-minute window (the '15 by 15' rule). This pattern reflects a well-oxygenated fetal central nervous system and autonomic reactivity.
10Under WHO's initiative for the triple elimination of mother-to-child transmission, pregnant women should be tested at their first antenatal visit for which three infections?
A.Toxoplasmosis, Rubella, and Cytomegalovirus
B.Measles, Varicella-Zoster virus, and Chlamydia
C.Syphilis, Hepatitis B virus (HBsAg), and Human Immunodeficiency Virus (HIV)
D.Helicobacter pylori, Brucellosis, and Rabies
Explanation: WHO's triple elimination initiative calls for all pregnant women to be tested for HIV, syphilis and hepatitis B (HBsAg), ideally at the first antenatal contact. Early detection allows benzathine penicillin for syphilis, antiviral therapy and infant birth-dose vaccination (with HBIG where available) for hepatitis B, and antiretroviral therapy for HIV.
