Frequently Asked Questions

How many questions are on the CSFA exam and how much time do I get?

The CSFA exam has 175 total multiple-choice questions: 150 scored and 25 unscored pretest items used only for statistical analysis. You get 4 hours to complete the computer-based exam at a PSI test center, and pretest items are indistinguishable from scored ones during testing.

What score do I need to pass the CSFA exam?

You must answer 99 of the 150 scored questions correctly, which is approximately 66%. The 25 unscored pretest items do not count toward your score, so treat every question on the exam as if it counts.

What happens if I fail the CSFA exam, and how soon can I retest?

NBSTSA sets no mandatory waiting period before reapplying, but candidates may test no more than three times per calendar year, and each attempt requires a new application and the full testing fee. Once you've used all three attempts in a calendar year, you must wait until the next calendar year to test again.

What is the CSFA pass rate?

NBSTSA's 2025 certification data reports a 62% first-time national pass rate for the CSFA exam, down from 71.2% in 2024 and 78.2% in 2023. This is noticeably lower than the CST exam's roughly 68-69% first-time pass rate over the same period.

How long is CSFA certification valid, and how do I renew it?

NBSTSA shortened the certification cycle from four years to two years, effective January 1, 2020. To renew, CSFAs must earn 38 AST-approved continuing education credits (including at least 8 live credits) within the current two-year cycle and submit the NBSTSA renewal application.

What are the CSFA content domains and their official weights?

Perioperative Care is 55% of the exam (83 of 150 scored items): Preoperative Preparation (8 items), Intraoperative Procedures (67 items, by far the largest single section), and Postoperative Procedures (8 items). Ancillary Duties is 14% (21 items: Administrative/Personnel 6, Equipment Sterilization/Maintenance 15). Advanced Science is 31% (46 items: Advanced Anatomy/Physiology 36, Advanced Principles of Microbiology 5, Surgical Pharmacology and Anesthesia 5).
